Mandatory Forms for Athletes
There are multiple forms that must be completed prior to your student participating in their first practice. These forms are mandatory and include:
- Physical Form
- Revised Concussion Information Release Form (*which was newly available on August 4th)
- Transportation Form
- Impact Test Letter & Form

All student-athletes that are planning to participate to sports are required by KSHSAA to be completed and returned to the school's office. For more information about immunization requirements, please visit the immunization web page.
